S.31 of the Dental Hygienists Profession Regulation requires that members provide a home address, telephone number, email address and fax (if available) when there are any changes to the information.
You will now be able to use your User ID and password at any time of the year to edit your personal profile. You’ll be able to update your home or email address, phone number, employer listing and more.
Continuing Competence Program Transcripts
With your User ID and password, you will be able to view and print your Continuing Competence Program Transcript whenever you like.
Please note that when you send requests for continuing competence program credits, you must allow the CRDHA office up to 120 days to enter the program credits in your personal record. Requests for credit will not be entered during the annual renewal drive in October – November of each year, except when the member’s three-year reporting period is ending.
Register for CRDHA Continuing Competence Events
CRDHA members can register and pay for upcoming CRDHA continuing competence events online.
Please note that CRDHA will not be hosting continuing competence opportunities this Fall. CRDHA office staff will be concentrating on the annual membership renewal, as well as relocating to another office space on December 1, 2010. For information on other continuing competence opportunities, please refer to the CRDHA website under Events.
ACCESS TO ONLINE RENEWAL WILL OPEN IN MID-SEPTEMBER, 2010
CRDHA is once again providing the option of online member renewal. If using online renewal, you can only renew in the same category of membership you currently hold or request cancellation of your registration. Transfer from General to Non-practicing membership or vice versa, must be completed by submitting the paper renewal application form.
Because General members must meet specific legislated requirements for renewal of practice permits, the online renewal system has a number of built-in parameters around those requirements. General members will only be able to renew online if information in the CRDHA’s database confirms that:
- the member’s CPR certification will be current on October 31; and
- the member has met the Continuing Competence Program requirements.
If the database does not contain a record of these two requirements being met, the online renewal process will not allow you to proceed. You will have to complete and mail the paper application form along with a photocopy of your CPR wallet card.
As a General Member you will be able to renew your membership online if:
- You have an email address;
- You are able to use Visa or MasterCard for payment of fees;
- You are renewing membership in the General member category; and
o You have submitted evidence of CPR certification (Health Care Provider Level - HCP) through the CRDHA Continuing Competence Program; and
o You have met the continuing competence credit and practice hour requirements if your three-year reporting period ends October 31, 2010; or
- You are requesting cancellation of your registration with the CRDHA.
As a Non-Practicing Member you will be able to renew your membership online if:
- You have an email address;
- You are able to use Visa or MasterCard for payment of fees;
- You are renewing membership in the Non-Practicing member category (including CDHA Support membership); or
- You are requesting cancellation of your registration with the CRDHA.
Accessing Online Renewal
To renew your membership online you will need a unique User ID and Password. You may change your password after you initially sign into the CRDHA member’s only site at www.crdha.ca. All passwords are case-sensitive. If you changed your password the first time you entered the CRDHA member site, you can continue to use that password. You are not required to create a new password each time you enter the site. For initial entry into the site, your unique identifiers will be:
User ID: Last Name + your 4 digit CRDHA ID number (no spaces)
Example: smithers9999
Password: Birthdate (yearmonthday) no spaces
Example: 19801001
CPR Certification and Online Renewal
The online renewal system will search your Continuing Competence Program records to confirm currency of your CPR certification. If you completed CPR certification 12 months or more prior to the renewal deadline of October 31, 2010, your CPR certification will not be considered current for the purpose of online renewal and the system will not allow you to proceed with online renewal. You will have to complete and mail the paper application form along with a copy of your CPR wallet card.
If you have recently completed your CPR certification, you can fax a copy of your wallet card along with a Request for Continuing Competence Program Credits and we will update your record so you can complete online renewal (please allow 3 business days for your records to be updated). Due to the increased workload during the renewal period, we will not be able to make updates to your continuing competence program records after September 30, 2010 unless your reporting period is ending October 31, 2010.
